Middle School Supply List

 

ALL SPORTS – PHYSICALS

Thursday, August 3rd at 6 p.m.
at the Football Stadium Field House

All sports & all athletes grades 7 – 12

Cost – $10

Physicals, concussion forms and insurance forms MUST be all completed before you can participate in any middle school or high school sports.

Pre-Enrollment for 2017-2018 School Year
for all Hugo School Sites

 

Wednesday, July 26, 2017     8 a.m. – 1 p.m. 

Thursday, July 27, 2017     1 p.m. – 6 p.m.

New Students must have their shot records.    All 7th & 8th grade students must have T-Dap shots.

STUDENTS CANNOT ENROLL UNLESS THEY HAVE ALL VACCINES----OKLAHOME STATE LAW


To enroll a new student or are transferring from a different school.

Please bring:

  • up to date immunization records
  • state issued birth certificate
  • Social Security card
  • A CDIB card if using JOM benefits
  • Copy of something showing proof of resident
